Treatment Guidelines
Medicare Health Support of Maryland and Washington, D. C. is a new, free, primarily telephone-based patient education and support program for selected fee-for-service Medicare beneficiaries with heart failure and/or diabetes.
Some of your patients may be among the 20,000 beneficiaries in the area who have been selected to voluntarily participate in this three-year pilot program. The program for Baltimore and Washington, D. C. is one of nine now underway across the country that were developed as part of the Chronic Care Improvement Program from the Centers for Medicare and Medicaid Services.
Should your patients come to you for advice about joining the program, we hope that you will encourage them to participate. In no way will the program interfere with the care plan you have developed, nor is it meant to replace office visits. Instead, the program is designed to reinforce your treatment plan and encourage patient compliance. Additional information about the program is available to providers by calling 1-866-807-4487.
