Program Benefits

Congratulations! You or your loved one has been invited by Medicare to participate in Medicare Health Support, a new, free program from Medicare for people who are living with diabetes, heart disease or both. This program is completely voluntary and does not interfere with your existing Medicare benefits, nor does it take the place of your doctor. You can choose to leave the program at any time. Medicare Health Support is an additional program from Medicare to help you follow your doctor's advice and feel better by providing you with health information and special services.

The Medicare Health Support nurses also work with your doctors to make sure they have all of the information they need to take care of your health needs. With your approval, the nurses are also available to answer questions from a family member or friend of your choice who may be helping you with your health care decisions.
